- Community Access for Disability Inclusion (CADI)
- Brain Injury (BI)
- Developmental Disabilities (DD)
- Rule 185 & VA/DD (Non-Waiver DD)
- Elderly Waiver (EW)
- Alternative Care Program (AC)

- Talk to your Hennepin County human services team or your current waiver case manager to request a transfer!
- Not yet on a waiver? Call the county at 612-348-4111 to inquire about getting access to waiver funding.
SERVICES INCLUDE:
- Coordination and assistance in managing waiver services.
- This service is funded by the Waiver program, and provided under the Minnesota Department of Human Services “Waiver Case Management“.
Case Management
Service planning, provider selection, and coordination of care provided for waiver-funded individuals.
