- Developmental Disabilities (DD)
- Elderly Waiver (EW)
- Alternative Care Program (AC)

- Not yet on a waiver? Call the county at 218-726-2000 to inquire about getting access to waiver funding.
SERVICES INCLUDE:
- Coordination and assistance in managing waiver services.
- This service is funded by the Waiver program, and provided under the Minnesota Department of Human Services “Waiver Case Management“.

Service planning, provider selection, and coordination of care provided for waiver-funded individuals.
