PROGRAM OVERVIEW
The Go Supportive Line delivers support to individuals living in their own home. This service is focused on increasing independence, preventing emergencies and escalations, and reducing the need for crisis intervention. Services include:

Proactive in-person and over the phone scheduled check-ins

Skill development

Post-hospitalization responses

Identification of natural supports

Support with locating & coordinating with care providers
