The Go Supportive Line service offers 24/7 over the phone and in-person response for problem solving just about anything.  Our team of responders come alongside to help bring peace in our client’s own home and life.  Whether it is help with managing stress and anxiety or navigating difficult decisions and life challenges, our Go Supportive responders are available any time, day or night.

PROGRAM OVERVIEW

The Go Supportive Line delivers support to individuals living in their own home. This service is focused on increasing independence, preventing emergencies and escalations, and reducing the need for crisis intervention. Services include:

  • Access to our team of responders 24/7
  • In-person responses when needed
  • Proactive in-person and over the phone scheduled check-ins
  • Skill development
  • Post-hospitalization responses
  • Identification of natural supports
  • Support with locating & coordinating  with care providers
The Go Supportive Line is available 24/7 over the phone to respond to these situations, and 7 days a week for in-person meetings.

  • 18 years of age or older
  • A mental illness, brain injury, or other disability
  • Funded through public waiver programs (CADI, BI, or DD waivers)
  • Living independently in their own home
