The goal of our program is to provide just the right amount of support to allow you to shift more of your time and energy toward achieving your full potential.

PROGRAM OVERVIEW

We deliver a variety of supportive services to individuals in their own homes. These services are focused on increasing independence, achieving personal goals, securing and maintaining housing, and reducing need for crisis intervention.  Services include:

  • Housing Stabilization
  • Waiver Case Management
  • 24-Hour Go Supportive Line (DHS “Emergency Assistance”)

QUALIFICATIONS

  • 18 years of age or older
  • A mental illness, brain injury, or other disability
  • Willing to take an active part in setting and achieving desired goals
  • Public funding available through public waiver programs (CADI, or BI waivers)
  • Those that do not qualify for public funding may pay privately.
